Consideration for Nurse III Self-Assessment Tool

You may complete your self-assessment with responses below, by attaching examples

of how you have met the requirements, or a combination of both. Attachments could

include power point presentations, process improvement projects, committee minutes,

or other documents that illustrate how you have met the requirement. Regardless of

how you choose to showcase your accomplishments, make sure that you fully address

each of the five dimensions within the nurse qualification standards: Practice,

Patient Driven Care, Leadership, Professional Development, and

Evidence-Based Practice/Research. You should be able to clearly identify the following

within each of your examples:

Action what specifically did you do? Did you identify an issue, lead a process

improvement initiative, participate in improving patient outcomes, etc.?

Population who was impacted by your action? Was it an individual patient, a

unit, a workgroup, a program, a service, a facility/ organization, or a region, etc.?

Outcome what difference did your action make? How did a performance

measure, nursing sensitive indicator, satisfaction, process, etc. change?

Your examples at the Nurse III grade should focus on improving outcomes beyond the

immediate practice setting. Examples of locations beyond the immediate practice

setting must be broad and complex such as multifaceted programs or services with

outcomes impacting the organizational level or beyond.
