Develop a proposal for an intervention to address your current issue of concern at your practicum site, and prepare a professional presentation to deliver to your peers. Submit your required practicum hours. Hours must be detailed per shift and confirmed by your preceptor before submitting your assessment. Papers submitted without detailed and confirmed hours will not be graded.

In Assessment 5, you will submit evidence of your detailed and confirmed practicum hours. You should have completed 40 hours by this time. You will also download and submit your four reflective journal entries.

In this assessment, you will develop a proposal for an intervention to address a current practice or issue of concern. You will also develop a professional presentation to deliver to your peers to convince your peers at your practicum site to adopt and implement your proposal.

Your assessment is in two parts:

  • Part 1: Intervention proposal:
    • 57 page scholarly paper.
  • Part 2: Narrated presentation to encourage adoption of the proposal, using Kaltura. Include:
    • No more than nine slides, not including the title and reference slide.
    • Title and reference slides.
    • A written, detailed Word document of the narration.

Part 1

In your intervention proposal paper:

  • Identify your practice issue of concern.
  • Identify current practice.
  • Propose strategy to improve current practice. What is the strategy, including the changes for people and process needed?
    • How does this enhance quality, safety, and reduce cost?
    • Describe the application of technology in your strategy for improvement.
    • What evidence are you basing your decisions on?
  • How would you implement this strategy at your practicum site? What challenges for implementation do you see at your site, and how would you overcome them?
  • What does successful interprofessional collaboration look like to implement this strategy? What interprofessional collaboration have or would you implement?

Part 2

Now develop a short, narrated presentation to make the actual change. Your presentation should explain to your peers at your practicum site and you should appear on video in professional attire:

  • The need for change.
  • The key aspects of your proposal.
    • Include the reason to implement it.

In the presentation, you’re selling your proposal and educating your peers on how to put it into action.

  • Your presentation must use Kaltura for the narration of the slides (and include you presenting your slides, if you choose to film yourself presenting). Do not submit a voiced-over PowerPoint. The file will be too large to upload.
  • Integrate relevant sources to support your arguments, correctly formatting citations and references using current APA style.
  • Limit it to no more than 7-9 slides of content, plus a title and reference slide. The presentation should go no longer than 10 minutes..
  • Include your written narrative or script of the presentation in a Word document.

Add the link to your video at the end of your written narrative.

Your assessment should meet the following requirements:

  • Length of paper: 57 page scholarly paper.
  • Length of video: 510 minutes.
  • References: Cite at least three professional or scholarly sources of evidence to support the assertions you make in your video. Include additional properly cited references as necessary to support your statements.
  • APA reference page: Submit a correctly formatted APA reference page that shows all the sources you used to create and deliver your video. Be sure to format the reference page according to current APA style.
  • Video and Narrative: You must submit a written narrative of all of your video content. Add the link to your video at the end of your written narrative.

  • You must submit the remainder of your practicum hours with this assessment. You should have completed a total of 40 confirmed hours with this assessment.
  • Hours must be detailed and confirmed or will result in a non-performance for this assessment.

Reflective journaling serves as a powerful tool for self-reflection during your academic journey. Reflective journaling enables you to assess your performance and to develop the regular practice of self-reflection. During your practicum, youll complete four reflective journal entries, each aligned with one of the four spheres of care.

Complete your first entry. You can choose any of the four spheres for this entry, but you will need to complete all four by the end of the course. Remember that youll need to submit your completed nursing reflection as part of the final assessment for the course.

Instructions

You will complete four reflective journal entries during your practicum, with an emphasis on the integration of professional standards and values in practice. Each entry will align with one of the four spheres of care.

As you complete your practicum hours (a total of 40 hours is required), it is good practice to complete a reflection journal entry for every 10 hours. AACN Definition of the Four Spheres of Care

Wellness and Disease Prevention: This sphere emphasizes health promotion, as well as the treatment of minor uncomplicated diseases or injuries for those not experiencing chronic illness or life-limiting conditions. Prenatal care, screenings, immunizations, and health promotion are some examples of care in this sphere (AACN, 2021). Nursing knowledge and competency in disease prevention and addressing social determinants of health are vital to promoting wellness across populations.

Chronic Disease Management: This sphere encompasses caring for those with one or more chronic diseases and preventing adverse outcomes associated with them. Specialized nursing care is often needed for this population due to the complex needs, along with integrated interprofessional team-based care. Four in 10 people in the United States have two or more chronic diseases, and those diagnosed with multiple comorbidities are projected to increase.

Regenerative and Restorative Care: This sphere includes critical and trauma care, complex acute care, acute exacerbations of chronic disease, and care of unstable patients who are typically in acute care hospital settings (AACN, 2021). This sphere includes the acute management of illness, such as a stroke or mental health crisis, and progression through the rehabilitative phase. Nursing skills and management of these populations are resource-intensive and specialized.

Hospice and Palliative Care: The final sphere relates to competencies surrounding palliative care in advanced illness and hospice care at the end of life. All registered nurses provide generalist palliative care in compassionate and patient-centered care while managing pain and other symptoms associated with advanced, progressive illnesses. The End-of-Life Nursing Education Consortium has mapped its competencies and recommendations for educating nursing students to the new AACN Essentials to assist educators in this endeavor.
